NOC for Name change in Kuwait Birth Card

Documents Required:
  • Passport and Civil ID copy.
  • One photograph.
  • Duly filled Form No 2.
  • Self-attested Passport and Civil ID copy of the parent/s.
  • Explanation letter regarding the changes required.

Please Note:

  • For Kuwait born, if any name change to be done in Passport, need to change the Kuwait Birth Card first.
  • For changing name in Birth Card, Embassy will issue NOC.
  • NOC may then get attested from MoFA and submit to the concerned authorities.
