Self-attested Passport and Civil ID copy of the both parents.
Photocopy of the Birth Certificate/ Passport of the child.
Passport Copy of the person who will be submitting passport application of child. Aadhaar Card Copy of the person in case they are not holding the passport.
Marriage certificate is required.
Please Note:
Both parents has to come in person.
The Applicant must produce Passport and Civil ID in original for verification.
Spouse name is mandatory in both parent’s passports if both parents are holding passports.
