  • Current Passport in original with photocopy (of first, last, address page (if separate page), any other endorsement pages, visa page and additional booklets, if any. For minors (up to 18 years) parents' passport copies (including endorsement pages) and visa copy are required.
  • Two valid ID card with the new name (space, sequence should be exactly same in both id) from India. This document can be from any Indian government related document which is mention the new name
  • The supporting photo ID or documents for name change should have the date of birth & father name matching as per the existing passport.
  • In case of MINOR applicant: Both parents original passport is required for verification. Also clear passport copy is required which includes the first, last & observation page if any and Valid UAE Visa page to be attached along with the application
  • News Paper advertisement in original, of two leading daily newspapers, notifying the change of name. One daily newspaper should be of the area of applicant's permanent address in Indian English and two daily newspapers in English and Arabic of the area of applicant's present address. Please attach full sheets of the newspaper with the advertisement and not cuttings thereof.
  • Both Newspaper advertisements should complete 7 days of notification period.
  • Draft a letter from the applicant stating why they are applying

    *Format of newspaper notification for MINOR is given below:- I______________________ father of_____________________, holder of Indian Passport No________, issued at _____________ on ________, permanent resident of ______________________ (full postal address in India to be given) and presently residing at __________________________ (full postal address in UAE to be given), do hereby change my son/daughter name from ________________ to ¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬___________________, with immediate effect.
